Sruti Bala Bhavam Series Participant Application

Dear Student/Parent,

Please complete and submit this application to present a 60-minute concert recital at a scheduled Sruti event in the future. Both the main artists and accompanying artists need to submit the form. Scheduling a student for a concert is up to the Sruti Board of Directors and all decisions made by the Sruti Board of Directors are final. The board will determine the accompanying artists for the performance. Please note that only children of Sruti Life Members are eligible to submit an application to perform. A non-refundable application fee of $100 should be submitted along with the application (payment link at the end of the form).
